Account Set-up Forms:
Credit Application: Complete this form and fax it to our New York office or to your local sales office to establish your credit, for open-account billing.
Refron Registry Enrollment Form: Complete this form and attach a copy of the EPA certification card for at least one of your employees, so that we can enroll you in the Refron Registry database, allowing your company to purchase regulated CFC and HCFC refrigerants.
Sales Tax Exemption Certificate: Complete this form and fax it to our New York office or to your local sales office, to establish your tax-exempt status. (If you are located in the State of New York or Washington, you must complete and fax in these forms instead: NY State Form or State of Washington Form)

Recovered Refrigerant:
Instructions for Shipping Cylinders Containing Recovered Refrigerant: Follow these procedures to make certain that your cylinders are properly filled, labeled and shipped, when returning used refrigerants to our reclamation facility.
Instructions for Shipping Drums Containing Recovered Refrigerant: Follow these procedures to make certain that your drums are properly filled, labeled and shipped, when returning used refrigerants to our reclamation facility. (For low-pressure refrigerants, such as R-11, R-113, R-123)

Return of Empty "Deposit" Cylinders:
Bill of Lading for the Return of Empty "Deposit" Cylinders: You may use this Bill of Lading form when returning empty "deposit-value" cylinders to our designated stockpoints or warehouses. (Make certain to call one of our offices to obtain the "Shipper's No / Bill of Lading#" to mark on your form when filling it out.)

Defective Cylinder Report & Return:
Report Form and Instructions for Claim of Defective Cylinder: If you have received a cylinder from Refron that you believe to be defective, complete this form according to the accompanying instructions and fax it to us. We will then contact you to arrange for an inspection. (Refron warehouses should use the Warehouse Defective Stock Form instead.)
